You are required to attach a signed, dated complete application form as a PDF (from the links below). In order to utilize Adobe electronic signature, you must first download and save the form to your computer desktop.

 Business Licenses:
Business Located in boundries of the City must submit:
  • Zoning Compliance Information Form click here 

Home based business must submit:

  • Certificate of Home Occupation Authorization click here
  • Signed permission from property owner if applicant is not the property owner

FAILURE TO PROVIDE ALL NECESSARY INFORMATION AND/OR DOCUMENTATION OR SUBMITTING THE FORM INCORRECTLY/INCOMPLETELY WILL RESULT IN THE DENIAL OF THE APPLICATION REQUEST.  IF THE REQUEST IS DENIED, CUSTOMERS MUST RESUBMIT A NEW APPLICATION WITH ALL NECESSARY DOCUMENTATION.  ONCE THE APPLICATION HAS BEEN ISSUED, ANY CHANGES MAY BE SUBJECT TO ADDITIONAL FEES.
